Membership Fees
What do I pay to Join?
You’ll pay a $99 activation fee, which covers your first month of membership. After that, your credit card will be charged $31 or $52 monthly, depending on your chosen plan.
Can I prepay my membership?
Memberships are currently on a month-to-month basis.
What details do I need to join?
You’ll need an email address for receipts, a credit or debit card, your full name, address, phone number, and date of birth.

How do I schedule preventive care?
Simply call our office to book an appointment for yourself or a family member.
How do I apply my member discount for treatments?
Let us know you’re a member when booking your appointment, and we’ll confirm your membership is active so you can enjoy savings.
How do I know if I need the Perio Membership?
Existing patients will receive confirmation for Perio Maintenance. New patients will be advised after their initial exam.

It’s your responsibility to reschedule. Please review our office policy regarding missed or last-minute canceled appointments.
Can I transfer my membership?
Memberships are non-transferable and valid only at our office. If you relocate, you can cancel your membership at any time.
